Parents/guardians can request an application by calling us at (860) 263-7535. We recommend printing a copy of the application (found on the right side of the page) to expedite the process. Note: If you are unable to print the application, please contact the school and we will send a paper copy to you.

Required forms:

 

1. Application Form– This form requests family and student biographical information and is to be completed by the parent/guardian.

 

2. Teacher Recommendations from 2 teachers – These forms are to be filled out by the fourth grade teacher and another recent teacher.

 

3. Transcript Release Form– This form should be returned to Grace Academy with the Application Form.

 

Additionally, students considered for admission into Grace Academy need to be eligible for the federal lunch program; therefore, proof of current family income is required for admission. 

We also strongly recommend attending one of our open houses as part of the application process.
